2025 NSPA Entry Form - Fill or Print
2025 Show Out of State Health Regulations:
Poultry who are NPIP:
-VS Form 9-3
-Pre-assigned out of state poultry permit
-Originate from NPIP flock or hatchery
Poultry who are NON NPIP:
-CVI (Health Certificate)
-Pre-Entry Import Permit Number
-Negative Pollurum and Typhoid test for each bird within 90 days prior to entry
